How to tie a knot for cleave gag

Hi, Do you know how to do a knotted cleave gag like in the photo below? Are there any tutorials about tying such knot?

Just a simple knot (in a scarf or a piece of cloth), and then place the knot on the mouth. Obviously another thight knot behind the head. Simple.
